Proposal Summary

The 90 min presentation starts with a 60 min Imagine YOU workshop. .Participants create an Image of Health that reflects what matters most to them in their lives and identify small steps they can be successful with to reach those goals. In the remaining 30 min they are trained to use the question " what matters most to you?" in their practice as a patient engagement tool.
The 3 1/2 hr training starts with the Imagine YOUworkshop as a self care exercise. Participants are taught the components of the workshop to use with their individual clients. Four key skills can be used in cliinical practice: Imagining, creating a card that represents what matters most to you, clarifying small action steps and making them small enough to be successful, getting support for your success.

I developed the Imagine YOU workshop and have presented it over 50 times in large and small group settings. We have trained over 50 Imagine YOU leaders to use the IY tools within their community organizations
